The cat efficiency code isn't being caused by the upstream sensor, but the downstream. The fact you've got a CVF catted DP is what's causing the rear O2 sensor to throw the code. Get a straight defouler and install it with the medium orifice, then thread the rear O2 sensor into it. That'll stop the P0420 code.

You can get the diaphram on its own online, its also a good time to check all rubber hoses. There is also a check valve on the turbo inlet, this is the high pressure (when your on boost) side of the system.
